Clinical Laboratory and/or Blood Bank Permit

          In New York State, clinical labs and blood banks need permits. The Wadsworth Center at the New York State (NYS) Department of Health gives these permits.
Labs run by health professionals for their own patients only do not need a permit, but they must register with the Wadsworth Center.
Facilities need the permit to:
- Test samples
 - Collect samples or blood
 - Distribute blood
 
Facilities that give patient-identified results need this permit too. These results may be for:
- Research
 - Testing
 - Toxicology
 - Identity testing
