 
              Certificate of Fitness to Supervise the Storage and Handling of Chemicals in NYC K-12 School Laboratories (D-15)
All laboratory specialists who store and handle chemicals in NYC K-12 Schools require a Certificate of Fitness for School Laboratories (D-15).
This Certificate of Fitness is valid only at a specific location and for the specific person to whom it is issued.
To obtain this Certificate of Fitness, applicants must be a NY State Permanent Certification as a Biology, Chemistry, Earth Science or Physics (7-12) Teacher or a NY State General Science (7-12) Teacher Extension Certification or a New York City Department of Education Certification as a laboratory specialist and have meet the special requirements. Qualified applicants should review the D-15 ALTERNATIVE ISSUANCE PROCEDURE INFORMATION (PDF). Applicants must apply online and provide the following information:
- Your name, mailing address, telephone number, email address, gender, social security number, date of birth, weight, height, and work location.
- A completed D-15 APPLICANT AFFIRMATION FORM (PDF). Individual applicants must have this form notarized.
- A NY State Permanent Certification as a Biology, Chemistry, Earth Science or Physics (7-12) Teacher OR a NY State General Science 7-12 Teacher Extension Certification OR a New York City Department of Education Certification as a laboratory specialist.
- A certificate or letter on the Department of Education’s official letter head stating that the applicant has obtained the DOE Science Safety training.
- A recent ID photo (2x2 color head shot) in JPG or JPEG format.
- A copy of your employee ID.
Online Applications:
- If the application meets all NYC Fire Department (FDNY) requirements, a certificate will be sent to the applicant's mailing address within five business days.
- If FDNY has any questions regarding the application, the applicant will be contacted within two business days. If the applicant fails to respond within 10 days, the application will be terminated. To reapply, applicants will need to submit a new application and pay the application fee again.
The NYC Fire Department (FDNY) will send a Renewal Notice 90 days before the expiration date. This letter includes the certificate number, the expiration date, and the amount of the renewal fee. It is the certificate holder’s responsibility to renew the certificate within 364 days of the expiration date. Certificates that are more than one year past their expiration date will not be renewed and a new application will need to be submitted. FDNY reserves the right to require applicants to take a re-examination upon submission of renewal applications. No certificate will be renewed without the required certification form or a retake of the examination.
To change your mailing address, submit the following in person or by mail to the NYC Fire Department (FDNY), Cashier’s Unit, 9 MetroTech Center, Brooklyn, NY 11201:
- A letter requesting the change of mailing address.
- A copy of your certificate with your old mailing address.
- A copy of your employee ID.
- A letter from your city agency on official letterhead to confirm that you are a current employee and that the Certificate of Fitness will be used as part of your work-related responsibilities.
To change your work location, submit the following in person or by mail to NYC Fire Department (FDNY), Cashier’s Unit, 9 MetroTech Center, Brooklyn, NY 11201:
- A copy of your certificate with your old work location.
- A copy of your employee ID.
- A letter from your city agency on official letterhead to confirm: 1) your new work location; and 2) that you are a current employee and that the Certificate of Fitness will be used as part of your work-related responsibilities.
To request a replacement certificate, submit the following in person or by mail to the NYC Fire Department (FDNY), Cashier’s Unit, 9 MetroTech Center, Brooklyn, NY 11201:
- A letter requesting a replacement certificate and stating your social security number and mailing address.
- A government-issued photo identification (such as a Driver’s License or Passport). The name on the ID must match the name on the certificate.
- A copy of your employee ID.
- A letter from your city agency on official letterhead to confirm that you are a current employee and that the Certificate of Fitness will be used as part of your work-related responsibilities.
