Certificate of Fitness for Use of LPG or CNG in Engine Fuel Systems (G-22)
City Fire Department
You need a Certificate of Fitness to use liquefied petroleum gas (LPG) or compressed natural gas (CNG) in engine fuel systems. These gases are extremely flammable, so you must follow strict safety procedures to reduce risk.
Many forklifts, often used in warehouses and on building sites, run on LPG or CNG.
This Certificate of Fitness is issued to one person only. You can use it anywhere in New York City.
To get this Certificate of Fitness, you must either:
- Pass a written test or
- Complete training from a company with a New York City Fire Department (FDNY) approved program.

If you need to take the exam, apply in person and bring:

Two forms of ID
- One must be a government-issued photo ID, like a driver's license or passport
If you completed FDNY-approved training:
Review the G-22 Alternative Issuance Procedure Information.
Only your company's Designated Coordinator can apply for you online. They must submit:
- Your personal details: name, mailing address, phone number, email, gender, Social Security number, date of birth, weight, height and work location
- A letter on company letterhead confirming you completed the training
- A completed Applicant Affirmation Form (PDF)
- A copy of your employee ID
- A recent ID photo (2x2 color headshot) in JPG or JPEG format
- A letter from FDNY confirming the training program is approved
Online applications
- If your application meets all New York City Fire Department (FDNY) requirements, your certificate will be mailed to you within five business days.
- If FDNY has questions about your application, they will contact you or your Designated Coordinator within two business days.
- If you or your coordinator do not respond within 10 days, your application will be canceled. To reapply, you must submit a new application and pay the fee again.
In-person applications
- If you pass the exam, you will get your certificate right after the exam.
- If you do not pass the exam, you will get a report that says you did not pass. To retake the exam, you must submit a new application and pay the fee again.
- The New York City Fire Code changes often. You must read and understand the latest Study Materials.
- This certificate must be renewed every three years.
Renewal Notice: The New York City Fire Department (FDNY) will send a Renewal Notice 90 days before your certificate expires.
This notice includes:
- Your certificate number
- Expiration date
- Renewal fee
You must renew your certificate within 364 days of the expiration date. If it is more than one year past its expiration date, you must submit a new application. FDNY may require you to retake the test when you renew. No certificate will be renewed without the required certification form or a retake of the test.
Change your mailing address
Submit the following in person or by mail to:

- A $5.00 personal check or money order, payable to: "Fire Department City of New York."
Change of work location
Submit the following in person or by mail to:

- A letter from your current employer (on company letterhead) confirming your employment and new work location
- If self-employed, submit a letter requesting a change to your work location
- A copy of your certificate with the old address
- A $5.00 personal check or money order, payable to: "Fire Department City of New York."
Request a replacement certificate
Submit the following in person or by mail to:

- A government-issued photo ID (such as a driver's license or a passport)
- The name on the ID must match the name on the certificate
- Your Social Security number
- Your mailing address
- A $5.00 personal check or money order, payable to: "Fire Department City of New York"
Pay the $25 application fee online using:
- Credit card (American Express, Discover, Mastercard or Visa)
- Debit card (Mastercard or Visa)
- E-Check
Note: A 2 percent fee applies to all credit card payments. The application fee is nonrefundable. If you are a City employee, you can request a fee exemption by submitting a copy of your employee ID and a letter from your City agency on official letterhead confirming that you are a current employee and that the Certificate of Fitness will be used for your work-related responsibilities.

Go to the Public Certification Unit with your completed application form and supporting documents. You will pay the application fee and take the exam.
No appointment is needed.

If you pass, you will get your Certificate of Fitness right away.
If you fail, you will need to submit a new application, pay the fee again and retake the exam.
Get your Certificate of Fitness Access ID
- Make sure you have your 12-digit Certificate of Fitness Access ID, which is on your Renewal Notice.
- If you do not have your Renewal Notice, your Access ID is your 8-digit Certificate of Fitness number and the last four digits of your Social Security number.
- If renewing for company employees, the company must be FDNY-approved and have an 8-digit Company Code
