The Paratransit Driver's License is required for an individual to drive a Paratransit vehicle that provides wheelchair-accessible transportation service in the City of New York.

The applicant must be at least 19 years old
A valid and original Social Security Card.
An original and valid Class A, B, C, or E Drivers License issued by the New York, New Jersey, Pennsylvania or Connecticut States Department of Motor Vehicles (DMV). The driver's license must include a current picture.
If the applicant's DMV license has a different address than the one put on the application, the applicant must bring proof of address (i.e., a current utility bill) that shows his or her current mailing address with their name on it.
Applicant is required to have an Active 19-A Commercial Driver License Status (Class A, B or C). Please see New York State Department of Motor Vehicles website link above.
An Examination to Determine Physical Condition of a Driver must be submitted with Doctors stamp. Under Article 19-A, the form cannot be older than one (1) year from the date application is submitted. For further information please visit the New York State Department of Motor Vehicles Link above.
The applicants may not have more than seven DMV points within an 18-month period, calculated from the date of application. Please see the DMV, NYC PVB, or NYC TLC Points Obligations link above.
The applicant must submit a completed and notarized NYC Office of Child Support Enforcement Form.
The applicant may not have any outstanding judgments (i.e. unpaid tickets or summons) to the DMV, NYC Traffic Parking Violations Bureau or the Taxi & Limousine Commission. If any outstanding judgments are discovered, the application will not be processed. To check for outstanding judgments with the DMV or PVB please contact those agencies directly. To confirm whether or not there are any outstanding judgments or open suspensions with the TLC please contact the TLC Call Center at 212 227-6324. Please have available the prior TLC license number, DMV license and or social security number to be assisted with a record search.

License renewal payments can be made online via the LARS (License Applications, Renewals and Summonses) System.
All other vehicle types licensed by the Taxi and Limousine Commission (TLC), including For-Hire Vehicles, Commuter Vans, and Paratransit Vehicles, require other individual operating licenses. All required documents listed above must be submitted, along with a completed application form, available at the link above. Applications can be submitted at either the Long Island City or Staten Island License Facilities, although visits to the Staten Island facility are by appointment, only. The applicant must fully comply with all requirements to complete the application within four months from the time it is submitted.

Fees must be submitted along with the application form. All fees are non-refundable.
The license application, required documents, and fees must be filed in person at a TLC Licensing center from Monday through Friday between the hours of 8:00am and 4:00pm.
