Each of the following documents must be submitted as an unlocked PDF file. All PDF files must be submitted in a simple PDF format with a digital signature as applicable. If files are submitted as a dynamic PDF document (with layers) or without the required digital signature, they will be returned for corrections, causing a delay in the review process.
Do not lock PDFs. The Rangehood Unit will need to stamp or mark up the documents during examination for approval. Ensure that Rangehood Plan PDFs are flattened. A flattened plan guarantees that the plan will be clear and legible for the Rangehood Unit. Please avoid free text boxes, markups etc. Each of the following documents must be submitted as separate PDFs.

As of 8/2/21, all Rangehood Applications including New Systems, System Upgrades and Amendments must contain DOB documentation which shows DOB approval for commercial cooking use of space. The following approved documents from DOB are acceptable for this purpose:
- Certificate of Occupancy
- Schedule A
- Letter of No Objection from Borough Office stating use of space.

Completed and signed TM-R2 application
Under Job Description and Location of Work to be performed, include a note describing the specific work to be completed, for example, “This Rangehood Application is for an upgrade installation for a manufacturer X-500 system”.

Rangehood Plan in 11x17 format stamped by a Licensed Master Fire Suppression Contactor (LMFSC), Professional Engineer (PE) or Registered Architect (RA).
Plans must include:
- Name of system manufacturer
- Current Materials and Equipment Acceptance (MEA) or Fire Department Certificate of Approval (COA).
- All information regarding control head, fusible link temperature setting, number and size of cylinders, nozzles, total flow points, flow points for each cylinder and cooking equipment, all dimensions of cooking equipment, ducts, hood and plenum are provided in tabulation form on the drawing.
- All parts and part numbers from the design manual included in tabulation form on the plan or a page of the design manual with all parts marked is attached with the plans.
- Clearance between open flame and deep fat fryers
- Type of piping, size and approximate length. (Length is subject to change within manufacturer specifications)
- Location of gas valve, type of gas valve, location and distance of manual pull station. A 3” x 5” space to allow for FDNY Stamping
- A note stating that plans are prepared as per 2022 Buildings Code Sections BC 904.1 and 2022 Fire Code sections 904.3.8.

Asbestos abatement compliance is required for new system / system upgrade submissions.
- DEP form ACP-5 if the scope of the work is not an asbestos abatement.
- A Certificate of Occupancy if the scope of the scope of the work is exempt from asbestos requirement
The Technology Management Unit does not accept the following DEP forms: ACP-7 & ACP-21.

Application Fees:
- New Application: $375 (If determined during the plan examination that this plan is considered a Complex Technical Analysis, you will be required to pay a "Complex Technical Analysis Fee" of $525)
- Resubmission Fees: No fee required if submitted within 6 months of Letter of Deficiency. After 6 months, application will be considered abandoned, and may require a new filing fee.
