Restaurant Wheelchair Friendly Decal

Food service establishments that qualify can get a Wheelchair Friendly decal to post at the front of their business. To get the sticker, you must fill out a survey. The survey will ask if your place is wheelchair accessible.
- Valid permit: Your restaurant must have a valid, current Food Service Establishment Permit from the New York City Department of Health and Mental Hygiene or be in the process of applying.
- Entrance requirements: The main entrance must be level with the sidewalk. If not, there must be a ramp, lift or other usable entrance for a wheelchair.
- Entrance width: The entrance must be at least 32 inches wide.
- Wheelchair dining spaces: Wheelchair dining spaces must be spread throughout the seating area. There must be a level route to those spaces.
- Email address: You need a valid email address if applying online.
- The Department of Health and Mental Hygiene will mail your decal to your restaurant within four weeks of your survey submission.
- New food establishments: If you are applying for a new permit, you can complete the survey online as part of your new application for a Food Service Establishment Permit.
- Existing food establishments: If you already have a Food Service Establishment Permit, you can still complete the survey online. You must create an account with the NYC Online Licensing system. From the list of available services, select Wheelchair Survey. You will need the CAMIS or Online Record number for your establishment.
- New food establishments: If you are applying for a new Food Service Establishment Permit, you must complete the Wheelchair Friendly Decal - New Food Service Establishment form, sign the disclaimer and submit the survey along with your permit application documents to the Citywide Licensing Center.

Complete the Wheelchair Friendly Decal - Existing Food Service Establishment form, sign the disclaimer and mail the survey to the NYC Department of Health and Mental Hygiene.
