Businesses must obtain applicable business certificates, based on their legal structure as described below. Although DCWP does not require hardcopy submission of business certificates, DCWP reserves the right to request and examine the documents under section 20.104 of the New York City Administrative Code.

• $5,010: License Fee for applicants who file for a Commercial Lessor License with aggregate rent of more than $100,000 that will be collected from non-profit organizations conducting charitable gaming activities.
• $2,510: License Fee for applicants who file for a Commercial Lessor License with aggregate rent between $50,000 and $100,000 that will be collected from non-profit organizations conducting charitable gaming activities.
• $75: Fingerprint Processing Fee per person.
• 2.49% of credit card payment amount: Non-Refundable Convenience Fee for paying with a credit card.

All Sole Proprietors, General Partners, Directors, Corporate Officers, and Shareholders owning 10% or more of company stock must be fingerprinted at the DCWP Licensing Center. More info

Each person who will be fingerprinted must bring valid photo identification (ID). Acceptable forms of ID include: a driver's license or a non-driver's ID issued by a State Department of Motor Vehicles; a license or permit issued by a government agency; a passport; an alien card / green card; or if applicant is a City, State or Federal employee, an employee identification card. EXCEPTIONS: Any person who currently has fingerprints on file with DCWP does not need to be fingerprinted. DCWP has fingerprints on file for anyone who provided fingerprints within the last three years and for those who provided fingerprints for an active DCWP license.

Sole Proprietorships operating under a trade or Doing-Business-As (DBA) name must file a Doing-Business-As / Assumed Name Certificate with the clerk of the county in which the business is conducted. Sole Proprietors doing business under their own name - and not under a trade or DBA name - do not need a Business Certificate.

If the premises will accommodate 75 or more persons, submit a copy of the Place of Assembly Certificate of Operation issued by the Department of Buildings (DOB).

Submit a copy of the Certificate of Occupancy, Temporary Certificate of Occupancy (expiring no less than 30 days from date of application submission), or Letter of No Objection from the Department of Buildings (DOB) confirming that the business' premises is suitable for the business. More info

If submitting a Certificate of Occupancy or Temporary Certificate of Occupancy, it must be the most current version and show approval of the premises for "Use Group 6" and "Bingo," "Bingo Hall," "Meeting Hall," or "Meeting Room."

Submit two completed and signed copies of NYS Gaming Commission application materials to conduct bingo games or games of chance. More info

 
- For bingo games, these documents include File Forms BC-101, BC-102, BC-102A, BC-116 LEASE Agreement, BC-204 Commercial Lessor's Monthly Rental Statement, and appropriate schedule forms (i.e., Form 103 Schedule D - Individual Statement; Form 104 Schedule B - Individual, Partner, Association Statement; Form 105 Schedule C - Corporation Statement; Form 108 Schedule E - List of Employees, Agents or Representatives.)

Corporations, Limited Partnerships, Limited Liability Companies, or Limited Liability Partnerships must register and remain active with the New York State Department of State (NYS DOS). Active status will be verified by DCWP prior to license issuance. Note: If operating under an Assumed or Doing-Business-As (DBA) Name, an Assumed Name Certificate must also be filed.

Businesses incorporated outside of New York State must obtain a Certificate of Doing Business in Good Standing from the Secretary of State from the business' home state, and file it with an application for Authority to Conduct Business in New York with the New York State Secretary of State.
