Radiation Producing Equipment - Certificate of Registration
If you own or use any radiation equipment for patient care, you need a Radiation Producing Equipment - Certificate of Registration from the NYC Department of Health and Mental Hygiene (DOHMH). You need this certificate to set up, keep or use the equipment. Registrations are sorted by facility and permit type. You must renew your registration every two years.

Pay outstanding fines: You must pay all fines for Health Code Violations before you can apply. You can pay in person at the Department of Consumer and Worker Protection (DCWP) Citywide Licensing Center or online.

Provide proof of insurance: You need to show proof of Workers' Compensation and Disability Insurance. The certificate must list the NYC Department of Health and Mental Hygiene (DOHMH) as the certificate holder.

If you do not need coverage, submit a signed Certificate of Attestation of Exemption (Form CE-200). -
Provide a Certificate of Authority to Collect Sales Tax: Show proof of a Certificate of Authority to Collect Sales Tax. The address on the certificate must match your address.

Complete the registration form: The form must list all the machines at your facility.

Private office applicants: If the facility is a private office, the named applicant must be a senior partner.

Corporate applicants: If the applicant is not a private practice office, the applicant is the corporate name of the facility. The application must be signed by an authorized representative, usually the president or CEO.

Mammography services: If the facility has a mammography machine or provides mammography services, include a copy of the Food and Drug Administration (FDA) certificate.

Inspection for dental, podiatric and veterinary facilities: These facilities must not use X-ray equipment on patients before inspection by the Office of Radiological Health or a Certified Radiation Equipment Safety Officer (CRESO).

Medical physicist report: All other facilities must work with a medical physicist to submit a report detailing the results of initial quality control tests on all radiation-producing equipment.

- Registration fee: $100
- Certified registration fee for radiation installations with linear accelerator(s): $600 (valid for five years)

Other regulatory requirements: Applying for a DOHMH permit does not satisfy the regulatory requirements of other City, State and federal agencies. It is the business owner's responsibility to meet all other requirements.
- If you applied online: You will get a receipt by email.
- If you applied in person: The Department of Consumers and Worker Protection (DCWP) will give you a receipt.

- Follow rules: All places using radiation producing equipment must follow the rules in Article 175 of the New York City Health Code.
- Certificate expiration: The Certificate of Registration expires two years from the end of the month in which your application was approved. You will get a reminder notice three months before it expires to renew your permit for the next two years.
- Pay fines: You must pay all fines for Health Code Violations before you can apply for or renew a permit. You can pay fines in person at the NYC Department of Consumer and Worker Protection (DCWP) Licensing Center or online. Visit DCWP to learn more. You can also pay fines online.

- Review documentation: Carefully review all documentation and forms required to register radiation producing equipment.
- Provide device information: When applying online, provide information about all radiation producing devices in the establishment.
- Gather supporting documents: Gather all supporting documents required to apply for this permit.
- Upload documents: Make individual electronic versions of each supporting document for uploading. Accepted formats: PDF, JPG, GIF, PNG, XLS, XLSX, DOC, DOCX, VSD and TXT.
- Create an account: Create an account with the NYC Online Licensing system. Payment is required to complete and submit the application. Partially completed applications may be saved and returned later.
- Enter information: Select the type of permit and enter the requested information into each field. Fields with an asterisk (*) are required. Upload copies of required documents individually.
- Make payment: Pay with a credit or debit card to complete the application.
- Schedule an appointment: In-person licensing and collections services are available by appointment only. Schedule an appointment to meet with a DCWP representative.
- Review documentation: Carefully review all documentation and forms required to register radiation producing equipment.
- Submit forms and payment: Gather all required documentation and complete the application form and required supplemental forms. Submit all forms, documentation and payment in person at the DCWP Citywide Licensing Center.

- New applications not accepted by mail: Applications for new permits are not accepted by mail.
- Register: You can renew online using the NYC Online Licensing system. You need to register first. Make sure to pay any fines before renewing. You can pay fines online.
- Use PIN: To renew online for the first time, you need the Personal Identification Number (PIN) from your Renewal Notice. The PIN links your records to one online account. You only need to link each permit or license record once.
- Get PIN: If you don't have your Renewal Notice, you can get your PIN by completing an online PIN Request Form or by calling 311.
- Upload documents: If you need to upload documents, you will be prompted to download the Microsoft Silverlight application.
