Small Animal Training Establishment Permit
If you train small animals for money, you need a permit from the Department of Health and Mental Hygiene (DOHMH).

Required documents checklist -
Insurance: you need Workers' Compensation and Disability Insurance. Show proof of coverage when you apply. If you are exempt, provide a Certificate of Attestation of Exemption (form CE-200) from NYS Workers' Compensation.

Sales tax certificate: show proof of a Certificate of Authority to Collect Sales Tax. The address on the Certificate must match your application address. Learn more about Sales Tax Vendor Registration (Certificate of Authority).

Animal care certificate: your establishment must have a manager who has passed an Animal Care and Handling Course. Show proof of the certificate or course registration when you apply. Certification is needed before opening. Learn more about the Animal Care and Handling Course.

Pay fines: pay all fines for Health Code violations before applying or renewing your permit. Fines can be paid in person or online.

Permit fees:
- Applications filed between January 1 and June 30: $70.00
- Applications filed between July 1 and December 31: $105.00*
*The fee for applications submitted between July 1 and December 31 includes the second half of the year and the following 12 months.

Get a receipt:
- If you apply online, you will get a receipt by email.
- If you apply in person, the Department of Consumer and Worker Protection (DCWP) will give you a receipt.
- Your business cannot start until it passes an inspection and the supervising manager gets the Animal Care and Handling Certificate. Learn more about the Animal Care and Handling Course.

Schedule an inspection:
- After you apply, call the Department of Health and Mental Hygiene (DOHMH) Bureau of Veterinary Public Health Services at 646-364-1783 to schedule an inspection.
- Your business needs to pass this inspection to get a permit.

Start operating:
- Once you pass the inspection and the supervising manager has the Animal Care and Handling Certificate, you can operate your business.
- You must show the permit and the supervising manager's Animal Care and Handling Certificate.
- All permits end on December 31 each year. You will get a renewal notice three months before.

Create an account: new users must create an account with the NYC Online Licensing system.

Gather documents: collect all required documents and make electronic versions for upload.

Select permit type: choose the permit type and fill in the requested information. Fields marked with an asterisk (*) are required.

Submit documents: submit the required documents with your application. You can save partially completed applications and finish them later.

Payment: payment is required when you apply.

Complete application form: fill out the standard application form and gather the required documents.

Power of attorney: if someone other than the business owner submits the application, a completed Power of Attorney/Authority to Act form is needed. The person must also show acceptable photo ID.

- Applications for new permits are not accepted by mail.
- Renew online using the NYC Licensing system: You need to register first. You cannot renew if you have unpaid fines. You can pay fines online before renewing.
- To renew online for the first time: You need the PIN from your renewal notice. You can link multiple records to one account using the PIN from each renewal.
- If you do not have your renewal notice: You can get your PIN by filling out an online PIN Request Form or by calling 311.
