To obtain this Certificate of Fitness, applicants must pass a written exam or receive training from a company with a NYC Fire Department (FDNY) approved training program. All applicants should review the Notice of Exam and Study Materials.

Applicants who need to take the exam must apply in person and bring the following documents: More info
- A completed CERTIFICATE OF FITNESS APPLICATION FORM (PDF)
- A letter of employment.

SAMPLE SELF-EMPLOYMENT LETTER (PDF) - Two forms of identification, at least one must be a government-issued photo identification, such as a driver's license or a passport.

Applicants who received FDNY approved training should review the G-22 ALTERNATIVE ISSUANCE PROCEDURE INFORMATION (PDF). Only your company's Designated Coordinator can submit an application on your behalf. Designated Coordinators must apply online and provide the following information More info
- Applicant's name, mailing address, telephone number, email address, gender, social security number, date of birth, weight, height, and work location.
- A letter on company letterhead to confirm the applicant has completed the training program.
- A completed APPLICANT AFFIRMATION FORM (PDF).
- A copy of the applicant's employee ID.
- A recent ID photo of the applicant (2x2 color head shot) in JPG or JPEG format.
- A letter from FDNY to confirm the training program has been reviewed and approved.

To submit an application, click on the Apply Online button. You must be authorized by the company to act as a Designated Coordinator and the company must be approved by FDNY. To request approval, email pubcert@fdny.nyc.gov.

Steps to apply in person:

Applicants must arrive at the Public Certification Unit ready to submit their application form and supporting documents, pay the application fee and take the exam. An appointment is not required.

Take the exam.

If you pass the exam, your Certificate of Fitness will be issued immediately. If you fail the exam, you will need to submit a new application, pay the application fee again and retake the exam.
