Instructions for applying for a private sewage disposal system permit -
Pay outstanding fines: Pay any fines for Health Code Violations before applying or renewing your permit. You can pay in person at the NYC Department of Consumer and Worker Protection (DCWP) Citywide Licensing Center or pay fines online.

- New Permit Application Fee: $1,310.00
- Annual Renewal Fee: $1,870.00

- Visa (online or in person)
- In-person submission: If you submit your application in person, the Department of Consumer and Worker Protection (DCWP) will give you a receipt.
- Online submission: If you submit your application online, you will get a receipt by email.
- Check application status: You can check your application status by calling the NYC Department of Health, Office of Public Health Engineering at 347-396-6001.
Renewing your permit
- You can renew your permit online using the NYC Online Licensing system. You need to register first.
- You cannot renew if you have unpaid fines. Pay fines online before renewing.
First-time online renewal
- Personal Identification Number (PIN): You need the PIN from your Renewal Notice.
- Linking permits: The PIN links your permits or licenses to one online account.
- Single-use PIN: Use the PIN only once for each permit or license.
Lost your renewal notice?
- Online PIN Request Form: Get your PIN by completing an online PIN Request Form.
- Call 311: You can also call 311 to get your PIN.
Uploading documents
- Document upload: You may need to upload documents.
- Microsoft Silverlight: Download the Microsoft Silverlight application if prompted.
Other permits and licenses
- Separate applications: Applying for a Department of Health and Mental Hygiene (DOHMH) permit does not cover other City or State requirements.
- Additional permits: You must apply for all other necessary permits, licenses and authorizations.
- Regulation compliance: Not following other regulations can lead to suspension of your DOHMH permits.
- Proof of compliance: Provide proof of compliance to avoid suspension.

Gather documents: Collect all the documents you need for the permit.

Make electronic versions: Create electronic versions of each document. You can upload PDF, JPG, GIF, PNG, XLS, XLSX, DOC, DOCX, VSD and TXT files.

Create an account: Sign up for the NYC Online Licensing system. Have your credit or debit card ready for payment. You can save and return to your application later.

Select permit type: Choose the permit type and fill in the required fields. Upload each supporting document individually.

Pay and submit: Use a credit or debit card to complete your application online.

Complete application form: Fill out and sign the Application for Permit.

Submit documents: Bring all completed forms and required documents to the DCWP Citywide Licensing Center. Applications are not accepted by mail.
