    Before you begin the application process, it is recommended that you contact the NYC Health Department, Office of Public Health Engineering to receive guidance on how to submit your supplemental documents and plans.

        If you do not have a NYC ID, please obtain one so you may use it on the MyCity Business portal to manage and track all of your interactions with the City of New York.

          All outstanding fines imposed for Health Code Violations must be paid before you can apply for or renew a permit issued by the NYC Department of Health and Mental Hygiene.

          Fines may also be paid in person at the NYC Department of Consumer and Worker Protection (DCWP) Citywide Licensing Center or Online. Appointments are required. Please use the email address below to request an apointment.

            This permit can be obtained only by a person who has proof of a degree in which chemistry, chemical engineering, or sanitary engineering was the major subject from a college or university approved by the Board of Regents of the University of the State of New York.

              The applicant must have proof of at least five years experience in the chemistry of water or in closely related work.
              • 7
                A corporation or partnership in which one of the officers or partners has the qualification required above must supervise all operations involving the addition of chemicals to water on a full-time basis.

                  Permittee must comply with all requirements outlined in Section 141.011 of the NYC Health Code about the equipment used and the chemicals allowed, including sampling requirements. To view the Health Code, go to www.nyc.gov/healthcode and select Article 141.

                    Permittee shall maintain or retain the services of a State certified laboratory equipped to analyze drinking water, in accordance with the latest edition of the Standard Methods for the Examination of Water and Wastewater. Prior to placing the system in operation, the permittee shall confirm that the drinking water supply, after being chemically treated, complies with Subpart 5-1 of the State Sanitary Code. Records of such analysis must be kept on file by the permittee for at least 5 years.

                      Proof of Worker's Compensation and Disability Insurance, including the name of the insurance carrier, the policy number, and the expiration date must be submitted when you apply for a permit and when you renew. Or, if exempt, a copy of Form CE-200 stamped by the Worker's Compensation Board with the New York State-assigned Exemption Certificate Number. 


                      The certificate must list the New York City Department of Health and Mental Hygiene as the certificate holder.

                            Gather all supporting or supplemental documents required to apply for this permit and make individual copies of each.

                            You will be uploading these documents to the online permit system.

                              Create an account with the NYC Online Licensing system. Payment is required to complete and submit the application so applicants should have their credit or debit card information readily available. Partially completed applications may be saved and returned to later for completion. You need to use the same email address as your NYC ID.

                                Select the type of permit for which you are applying and enter the requested information into each field. Fields with an asterisk (*) are required. Applicants will be requested to upload copies of required documents. Please upload each supporting document individually.
