Tanning Indoor Facility Permit
You need a NYC Health Department permit when ultraviolet (UV) radiation tanning devices (tanning machines) are used, offered or made available for use.
Places like tanning parlors, salons, gyms, health clubs, apartment buildings, condos, country clubs and hotels are required to have this permit if they have a tanning machine.
You do not need this permit if a qualified health professional uses UV radiation devices to treat medical conditions.

- Permit application fee: $120
- Inspection fee: $200 for each UV unit
- Biannual renewal fee: $120 plus $200 for each UV unit

- If you turned in your application in person, the Department of Consumer and Worker Protection (DCWP) will give you a receipt.
- If you turned in your application online, you will get a receipt by email.
- You can check your application status by calling the NYC Health Department, Office of Public Health Engineering at 347-396-6001.
- All tanning facility operators are required to put a disclaimer on their websites. NYC Health Code section 177.11(g) requires that any webpages that list, advertise, display, or allow booking of UV tanning services must contain the following message, which must be clearly displayed in minimum 10-size font with contrasting color:
- UV radiation from indoor tanning devices can cause:
• Skin Cancer, including melanoma, the type of skin cancer responsible for the most deaths
• Eye burns that can cause intense pain and negatively affect vision
• Sunburn (discomfort, pain and tenderness on the skin)
• Early skin aging, such as wrinkles and age spots
- UV radiation from indoor tanning devices can cause:
- All tanning facility operators must follow Article 177 of the NYC Health Code.
- Operators must give every customer a copy of the NYC Health Department fact sheet about the health risks of indoor tanning.
Indoor Tanning Health Risk Information for Customers (PDF) - Every year, operators must have every customer read and sign a NYC Health Department Indoor Tanning Patron Acknowledgement Form. The operator or an employee must also sign the form. Keep a current signed form on file for all customers using a tanning device.
- No one under age 18 can use a tanning machine (UV device). Operators must check a government-issued photo ID for all customers age 18 or older.
- The operator must report all injuries or illnesses from using a UV device to the NYC Health Department's Office of Radiological Health within 24 hours. Reportable injuries and illnesses include eye injuries, burns and any other illness or injury needing medical care. Keep all injury reports on file for two years.

Gather documents: Collect all the documents you need to apply for the permit.

Provide UV device information: When applying online, you need to give details about all UV devices in your facility.

Create an account: Sign up with the NYC Online Licensing system. Have your credit or debit card ready for payment. You can save and return to incomplete applications.

Pay and submit: Use a credit or debit card to complete and submit your application.

Complete forms: Fill out the Application for a Permit and the Supplemental Form (including UV Device Manifest). Gather all required documents.

Submit documents and payment: Bring the completed renewal notice, worker's compensation information, other required documents and payment to the DCWP Citywide Licensing Center.

You will receive a renewal notice three months before your permit expires. Every time you renew your permit, you will need to update the device information (manifest) for all UV devices in your establishment, if applicable. If you renew online, you can enter this information when you renew your permit. If you renew in person, you must submit the separate manifest sheet you received with your renewal. Payment for the permit fee and each device must be submitted.

Renewals may be submitted online via the NYC Online Licensing system. Registration is required. You will not be permitted to renew a permit if you have outstanding fines. You can pay fines online before renewing.

To renew online for the first time, you will need the Personal Identification Number (PIN) found on your Renewal Notice for each permit or license you hold. The PIN will allow you to link your existing records to one single online account. You only need to use your PIN to link to each permit or license record once.
If you do not have access to your Renewal Notice, you may obtain your PIN(s) by completing an online PIN Request Form or by calling 311.

- You may renew your permit in person at the DCWP Citywide Licensing Center. If you send someone to renew for you, they must present an Authority to Act on your behalf.
