Before you submit your application

    Submit a copy of the business' Surety Bond (or Third Party Bond), properly signed. More info

      The copy of the bond must name the City of New York as Obligee or the party to whom the bond must be made payable. Your corporate name, Doing-Business-As (DBA) name (if any), and premises address must be exactly the same on all documents. Applicant must also submit a copy of the bond receipt showing that the bond has been paid in full and does not expire before the end of the licensing period. The amount of the bond depends on the number of employees the business has.

      • for 0 to 25 employees: $500
      • for 26 to 50 employees: $1,500
      • for 51 to 75 employees: $2,500
      • for 76 to 125 employees: $3,000
      • for 126 or more employees: $5,000


  • 6

    Provide proof of workers’ compensation insurance. More info

       Acceptable proofs include:

      • C-105.2 – Certificate of Workers’ Compensation Insurance OR
      • U-26.3 – State Insurance Fund Certificate of Workers’ Compensation Insurance OR
      • Equivalent or successor form used by the New York State Workers’ Compensation Board

      If you are not required by law to maintain workers’ compensation insurance, you must submit form CE-200 – Certificate of Attestation of Exemption.

    Provide proof of disability benefits insurance. More info

      Acceptable proofs include:

      • DB-120.1 – Certificate of Insurance Coverage under the NYS Disability Benefits Law OR
      • Equivalent or successor form used by the New York State Workers’ Compensation Board


      If you are not required by law to maintain disability benefits insurance, you must submit form CE-200 – Certificate of Attestation of Exemption.

    Provide proof of commercial general liability insurance. More info

      Note:

      • Your Certificate of Insurance must name the City of New York as the certificate holder.
      • Your Certificate of Insurance must be executed by a broker and list the National Association of Insurance Commissioners (NAIC) Number.
      • Your business name and address must be exactly the same on all documents.
      • The City of New York must be included as “Additional Insured” in the description box.
      • Insurance must have A.M. Best (www.ambest.com) rating greater than A- / “VII” or a Standard & Poor’s rating of at least A.
      • The policy must provide coverage that is at least as broad as the most recently issued edition of Insurance Services (ISO) Form CG 0001, must be “occurrence” based, and have the following minimum amounts:
        • $1,000,000 per occurrence
        • $2,000,000 aggregate

    Provide additional insured endorsement. More info

      Note:

      • DCWP accepts the following Additional Insured Endorsements:
        • ISO Form CG 20 26
        • ISO Form CG 20 12
        • An endorsement that provides coverage at least as broad as the most recent edition ISO Form CG 20 26 or CG 20 12.
      • If you are providing a Scheduled Additional Insured Endorsement that lists the additional insured name (i.e., ISO Forms CG 20 26 or CG 20 12), the endorsement must include either of the following phrases:
        • “The City of New York including its officials and employees.” OR
        • “The City of New York together with its officials and employees.”
      • If you are providing a Blanket Additional Insured Endorsement or an Automatic Additional Insured Endorsement, confirm that the coverage is at least as broad as the most recent edition of ISO Forms CG 20 26 or CG 20 12 before you submit the form.

    Submit a Notarized Certification by Broker or Agent.

      If the license application will be filed by someone other than the applicant, complete the Granting Authority to Act Affirmation.

        License Fees are pro-rated depending on the time the application is filed and the number of individuals employed by the business. More info

        You must have the applicable Business Certificate based on your business’s legal structure. More info

          Although DCWP does not require submission of your Business Certificate in order to process your application, DCWP may request this document under section 20-104 of the New York City Administrative Code.

        Complete the Basic License Application. More info

          The Basic License Application requires business information, including applicable identification numbers (such as EIN, SSN, or ITIN); contact information for the business location and business owner, history of criminal and / or civil charges for the business owners, partners, shareholders, and officers; and any DCWP license history for individuals named on the application and their relatives.

        Complete the Industrial Laundry Self-Certification. More info

          The owner (e.g., sole proprietor, general partner, director, corporate officer, or shareholder owning 10% or more of company stock) must complete the Self-Certification. Reminder: If your business will engage in Industrial Laundry Delivery services, you must complete the Vehicle and Delivery Information section of the Self-Certification.

        The business must provide their New York State Sales Tax Identification Number or Certificate of Authority Application Confirmation Number on the Basic License Application.

          Steps to apply online:

          Review the requirements for the license and gather all materials before clicking the Apply Online button. Note: You will be redirected to a new web page where you will need a registered account with the City of New York to submit an online application.

            After submitting an application online, print a copy of the Confirmation Page. The Confirmation Page will list the additional steps and materials required to complete the application. These materials are also listed above and must be submitted within five business days of filing the application online if they were not uploaded as part of the online application submission.
