Food service establishments that qualify can obtain a Wheelchair Friendly decal to post at the front of their business. Applicants must complete a survey attesting to the establishment being wheelchair accessible.

Description
The food service establishment must have a valid, current permit issued by the New York City Department of Health and Mental Hygiene, or be in the process of applying for a permit.
LEARN MORE
The establishment must have a main entrance flush with the sidewalk. Or, there must be a ramp, lift or other usable entrance for a wheelchair.
The entrance to the establishment must be at least 32 inches wide.
Wheelchair dining spaces must be distributed throughout the seating area and there must be a level route to those spaces.
A valid e-mail address is required if applying online.

Description
New Food Establishments: If you are applying for a new permit, you may complete the survey online as part of your new application for a Food Service Establishment Permit.
LEARN MORE
Existing Food Establishments: If you already have a Food Service Establishment permit, you can still complete the survey online. You must create an account with the NYC Online Licensing system. From the list of available services, you will select Wheelchair Survey. You will need the CAMIS or Online Record number for your establishment.
