Therapeutic Radiation (LINAC)- Certified Registration of Units
The Department of Health and Mental Hygiene (DOHMH) gives Certified Registrations. This lets you use therapeutic radiation units (LINACs) to treat people.
If you work at a medical institution, only the management can apply.
If you work elsewhere, any professional practitioner can apply.
After you get a Certified Registration, you must:
- Follow the statements, representations and procedures in your application and correspondence with the Office of Radiological Health (ORH).
- Follow the terms and conditions of the Certified Registration.
- Follow Article 175 of the New York City Health Code.
All places that use radiation therapy machines must get a Certified Registration. You must also pay the fees listed.

If you want to make a change that affects your operation or facility or that will cause the information on file with the DOHMH to be out of date:
- Request and get an amendment.
- Pay a fee before making the change.

Review forms and documents: Check the forms and documents needed to apply for a permit to operate a therapeutic radiation unit or medical linear accelerator (LINAC).

Pay outstanding fines: Pay all fines for Health Code Violations before applying or renewing a permit. Fines can be paid:
- In person at the NYC Department of Consumer and Worker Protection Citywide Licensing Center (appointment required)
- In person or online at the NYC Department of Finance
- Online at MyCity Business. Select Violations and then Pay Your Violation

List of fees:
- $600 for certified registration valid for five years
- $235 for each amendment to any certified registration
- Note: Do not submit payment with your application. You will be billed after your application is reviewed and approved.

Proof of insurance: Submit proof of Worker's Compensation and Disability Insurance, including the insurance carrier's name, policy number and expiration date.
- The certificate must list the New York City Department of Health and Mental Hygiene as the Certificate Holder.
- If your business is exempt from this insurance, get a Form CE-200 stamped by the Worker's Compensation Board with the New York State-assigned Exemption Certificate Number.
- Workers' Compensation Insurance and Disability Benefits Insurance
- Initial application: After you send in your first application, you will work with someone from the NYC Department of Health and Mental Hygiene, Office of Radiological Health (ORH). They will help you finish the application. You can call ORH at 718-786-6002 or email them at orh@health.nyc.gov.
- Application approval: Once your application is checked and approved, you will get a bill in the mail for the registration fee. You must pay the fee before you are issued your permit.

Applying for a DOHMH License: Applying for a DOHMH License for Therapeutic Radiation units does not cover all regulatory requirements. You must follow other city, state and federal rules, too. If you do not, your DOHMH permits may be suspended until you show proof of compliance with all rules.

When renewing: You must show proof of Worker's Compensation and Disability Insurance. This includes:
- The name of the insurance company
- The policy number
- The expiration date of the policy

Renew on time: You must renew your certified registration on time. Complete an application and mail it to the address below 30 days before your permit expires. After your application is reviewed and approved by the NYC Department of Health and Mental Hygiene, Office of Radiological Health staff, you will get an invoice. You must pay the invoice before your registration is renewed.
